Does USPS require last names on mail to deliver it? Or is a first name enough?

That might raise some suspicion. I've had mail delivered to me before with just a initial as last name. Ex. Josh B. Nothing illegal though just reg mail and it has worked fine.

I received mail addressed with "Uncle Josh" handwritten from my niece. FWIW.

I always recommend to my customers to give me their Mr. Ms. Mrs., first initial, then a weird spelling of their last name or a name that is very similar. That way if you really feel like it you can keep the package laying around unopened for a couple of days and if anything were to happen you have plausible deniability.
If you live in an apartment building that has a high turnover rate just put whatever name you want on there because the postman is really only concerned with the box # (this only if you have been living there a short while, not if you have been receiving mail for 4 years there or something).
Last initial would work too, but again you want your mail to be completely the same as all the other corporate mail out there. Corporations mis-spell last names all the time, or put wrong last names etc that are simliar because the person taking the information on the phone wasn't listening. Packaging itself is equally important as addressing. No odors, no leaks, no bumps, etc.

Packages should be as ordinary as possible. Make them look like all your other mail not weird, packages that stand out or get noticed are the very worst thing you can do and are the most risky.
